What to bring to your appointment

1. A written referral - Please make sure that you have a written referral from your doctor or HMO. If you don’t have one, please call us at (719-564-1542) to determine if you need to obtain one. Most insurance companies require us to obtain written authorization BEFORE examining their customers. Almost all HMO's and Medicaid require us to have a written referral before we see you as a patient.
2. Your insurance cards - Please make sure you bring all of your insurance cards to your visit:

   •	Your primary insurance cards
   •	Your secondary insurance cards
   •	Any Medicare and/or Medicaid cards

3. A current chest x-ray and all available previous x-rays. A current chest x-ray is required for all new patients and for most returning patients. If your referring physician did not obtain a chest x-ray, please call us to schedule one before your doctor visit.
4. A list of medications. Please bring a list of all medications, along with all pill bottles, including any vitamins, minerals, or herbal medications.
5. HMO Co-payment. If you are a member of an HMO, we expect your co-payment at the time of your visit.
